We are currently recruiting a field staff Supervisor for Stratford/Perth.
Reporting to the Care and Services Manager, this Supervisor is responsible for the overseeing the Personal Support Worker field staff where service is provided by the District, ensuring delivery of high quality services to meet client needs.
Qualifications Required:
-RN or RPN with current registration with College of Nurses of Ontario (CNO)
-Preference will be given to candidates with two (2) or more years of supervisory experience
-Current CPR certificate; current Driver’s License and vehicle
-Home health care experience an asset
Responsibilities:
Client assessment: Includes meeting with clients to conduct both medical and safety assessments; creating documentation on client case; developing care plans; preparing recommendations for client care, resources, additional services; reporting any changes in client needs to Community Care Access Centres (CCAC) or other funding sources; conducting periodic follow-up with clients.
Advocating for clients: Communicating client needs to healthcare stakeholders; justifying and securing resources; reporting on abuse or unsafe conditions.
Liaise with external customers: Includes participating in joint case management with healthcare stakeholders; taking action to address client complaints and problems.
Supervision: Field staff supervision, performance management; site visits; providing support and advice for field staff.
Candidates successful in obtaining employment will be required to provide a Criminal Record Check with Vulnerable Sector Screening valid within six (6) months of start date, as well as proof of TB Mantoux skin test completed within twelve (12) months of start date.
