COORDINATOR RESIDENT CARE (SSH)

REPOSTED

Length:  Temporary Full-time (For approximately 12 months)

Salary: $81,062.80 – $101,319.40 per annum/$44.54 – $55.67 per hour

Location:  Sunnyside Home, 247 Franklin Street, Kitchener

Description of Duties:

Ensures that the complex physical, psychological, spiritual, and social needs of residents are met in an interdisciplinary team environment with a focus on customer service and a restorative care philosophy.  Supervises the work of Team Leaders. Performs performance management for approximately 10 Registered Nurses. Supports Team Leaders in the coordination, assigning, and performance monitoring of a multi-disciplinary team.  Accepts on-call responsibilities approximately 6-8 weeks per year.  Work is performed to meet inflexible deadlines and is subject to peak periods.

Current Assignment:

 
  • Leads the planning, coordination and evaluation of risk management, quality improvement and client safety programs for the Seniors’ Services division. Coordinates Seniors’ Services participation in accreditation processes. Assumes the duties and responsibilities of the Administrator, Resident Care (Sunnyside Home) as required. Develops, implements, monitors and leads the Division’s Quality Improvement Program, including coordinating planning, training and auditing to ensure compliance with the Long term Care Homes Act, 2007 and related regulations. Develops annual quality plan to ensure the division meets requirements of the Residents’ First provincial initiative of Health Quality Ontario and the Resident Quality Inspection by the Ministry of Health and Long Term Care. Leads the division’s Quality Improvement & Risk Management and Privacy committees.

Knowledge, Skills & Abilities Required:

 
  • Comprehensive knowledge of Resident Care and gerontological nursing, psychogeriatric and palliative care, long-term care, recreational and physical therapy, and community resources, normally acquired through the attainment of a Bachelor of Science Degree in Nursing, or equivalent education in the Health Sciences disciplines, and current registration with the College of Nurses of Ontario as a Registered Nurse.
 
  • A tuberculosis (TB) test and annual influenza immunization are required.
 
  • Knowledge of Regional policies and procedures and related legislation, including Ministry of Health and Long Term Care Program Standards, Occupational Health & Safety Act, Mental Health Act, Homes for the Aged Act, Municipal Freedom of Information and Protection of Privacy Act, Personal Health Information Protection Act, and other related legislation.
 
  • Assessment, analytical, and problem solving skills to plan and develop objectives including administrative, technical, operational, and financial components normally acquired through a certificate program in management and progressively responsible management experience. Ability to organize and coordinate staff activities to achieve established goals and objectives. Ability to research and develop education programs.
 
  • Human relations and communication skills in order to mentor, advise, and supervise staff in a multiple union environment; resolve conflict; respond to complaints and inquiries from residents, families and/or designated decision makers, using diplomacy and tact; and to participate as an effective team member.
 
  • Computer skills, using software such as Microsoft Office, Point Click Care, Microsoft Outlook, and a time and attendance system.
 
  • Must provide an acceptable Police Records Check in order to ensure suitability for working with vulnerable populations.
 
  • Ability to read and interpret journals, books, newsletters, correspondence, charts, reports, resident files, policy and procedure manuals, collective agreements, staff schedules, and employment applications in order to communicate to staff as required. Ability to compose and write reports, minutes, staff schedules, labour relations documentation, and performance appraisals, as well as to develop policies and procedures.
 
  • Ability to travel to various locations within the Region.
 
  • Ability to support and project values compatible with the organization.
